You represent that the mobile phone number(s) you provide belong to you and that you have the authority to consent to receive text messages at those numbers. 11. Indemnification

To the extent any dispute is not subject to arbitration, such dispute shall be brought exclusively in the state or federal courts located in Arizona.

16. SMS Messaging Terms

a. Program Description
URPrecious Imaging uses text messaging (SMS/MMS) to communicate important information such as appointment reminders, scheduling updates, exam preparation instructions, patient portal alerts, limited billing notifications, and patient experience follow-ups.

b. Consent to Receive Messages
By providing your mobile phone number and opting in through our Site, intake forms, or patient portal, you consent to receive SMS/MMS messages from URPrecious Imaging. Your consent is not a condition of receiving care or services, and you may obtain services even if you choose not to receive text messages.

c. Opt-Out / STOP Instructions
You may opt out of receiving text messages at any time by replying STOP to any URPrecious message. After you send STOP, we may send one final message to confirm that you have been unsubscribed. You will no longer receive SMS messages from that specific program, but we may still contact you by phone, mail, email, or through the patient portal as permitted by law.

d. Help / Support Instructions
For help regarding our text messaging program, reply HELP to any URPrecious message or contact our office directly at 602-878-7501 during normal business hours.

e. Message Frequency, Carriers, and Fees
Message frequency may vary depending on your appointments and interactions with URPrecious Imaging. Message and data rates may apply. Not all mobile carriers or devices may support our messaging program, and URPrecious Imaging is not responsible for delays or failures in message delivery. Carrier participation and coverage may change without notice.

f. Protected Health Information and Privacy
Text messages may contain limited information related to your care (for example, appointment date, time, and location, or basic prep instructions). We strive to limit sensitive details in SMS messages; however, text messaging is not a fully secure form of communication. By opting in, you acknowledge and accept these risks. For highly sensitive information, please use our patient portal, secure channels, or speak with us directly.

g. Changes to Messaging Program
URPrecious may modify or discontinue its SMS messaging programs at any time, with or without notice, as permitted by applicable law and carrier requirements.
